Professional and Community Education on Having the End-of-Life Conversation / by Joe Furia

AzHHA’s Thoughtful Life Conversations program is offering professional and community engagement and activation trainings throughout this summer. Two of the trainings offered are opportunities to have someone within your organization or community to be trained to facilitate the Honoring A Life: Advance Care Planning Conversations (for community members) and Communication in Serious Illness (for healthcare professionals) workshops.

The Honoring A Life: Advance Care Planning Conversations workshop for community members is a 2-hour workshop that helps people begin the conversation and outlines a clear process for them to ensure that their priorities and preferences for end-of-life care are known, documented and honored. We are also offering professional education for healthcare providers with a 2.5-hour didactic session that reviews the need for a systematic approach to having more, better and earlier conversations about patient values and priorities in serious illness. Providers will learn how to define a population with serious illness who may benefit from the serious illness conversation and explore alternative methods to improve communication in patients with serious illness with a structured communication tool.
