This 1.5-hour workshop provides resources for healthcare professionals on Arizona laws, which forms to use and outlines a clear process to communicate effectively with their patients about the value of advance care planning. While no one likes to talk about dying, this workshop provides an evidence-based approach and examples of proven communication strategies that result in completion of advance directives. Objectives are to define the process of advance care planning, review the evidence of effective messaging, discuss terminology, statutes, and forms, discuss how to approach the conversation and overcome barriers; review how to document the conversation, and discuss when to review and update these documents.
