The target audience is healthcare professionals who plan on using POLST or caring for patients/residents who have a POLST in all care settings.

This session has been approved by:

AAFP- American Academy of Family Practice- 1.0 elective credit, California Board of Nursing- 1.0 contact hour, National Association of Social Work- AZ Chapter- 1.0 contact hour, Nursing Care Administrators and Assisted Living Facility – 1.0 hour.

We are waiving the $25.00 fee for this workshop due to COVID-19.
