Join the AHA Age-Friendly Health Systems Action Community

American Hospital Association (AHA) is currently recruiting hospital teams to join the Age-Friendly Action Community, convening online this September. The Action Community is a free 7-month virtual collaborative designed for hospital-based teams, ambulatory care and long-term care facilities to test and implement the 4Ms Framework (Mobility, Mentation, Medication, and What Matters) and share learnings with their peers. Over 180 sites of care joined in the AHA’s first Action Community, and 75 sites achieved a level of age-friendly recognition.

Benefits of joining the action community include:

  • No limit to the number of hospital sites/team who would like to enroll. Hospitals may elect to enroll two ICU teams and an ED team.
  • Testing and learning are designed to occur as part of each team’s existing activities, therefore, a re-purposing of time rather than additional time.
  • Be part of a national movement to become a designated Age-Friendly Health System.

The AHA will be hosting an informational webinar on August 19 for those who are interested in learning more about the Action Community. Teams can enroll by completing the registration form.
