Session: Building the Integrated Behavioral Accountable Care Organization for All Arizonians

Thursday, Oct. 20 │ 1:15 p.m. breakout session: Health Equity

The ONEcare Network model assembles high-performance, clinically integrated behavioral health care teams that include hospitals, social services, inpatient behavioral health, and primary care providers with the goals of enhancing disease screening and monitoring, improving treatment engagement and adherence, and decreasing the stigma of mental illness and substance use disorders. Using the 3 P’s of health equity (people, place, and partnerships), these networks create integrated systems of care that magnify the impact of a single provider as well as build partnerships to address the social determinants of health inequities to deliver the right care, in the right place, at the right time. Join members of several ONEcare Networks in Arizona to hear how they are improving access to care and positively impacting engagement and health outcomes.
